MSHSAA Medical Release and Parent Permission Form The physical exam must be dated post-Feb. 1 of the current calendar year to be valid for fall soccer.  NO PLAYERS WILL BE ALLOWED TO TRYOUT WITHOUT A VALID PHYSICAL.

SMTC Registration/Waiver Form The form must be completed and signed by a parent and submitted to SMTC staff before a player will be eligible to participate in performance training.

WGHS Soccer Summer Training Registration Form The form must be completed and submitted by the deadline provided. Please retain the top portion for reference during the summer.

WGHS Soccer Player and Parent Information Form The form should be completed annually to ensure accurate biographical and contact information for each player
